Medicaid work requirement in Ohio: what applies to you in 2027

Starting no later than January 2027, most adults 19 to 64 on Medicaid expansion must show 80 hours a month of work, school, volunteering or a work program, or $580 of income, unless the rule does not apply to them. What Ohio has published

When it starts
Requirement effective 2027-01-01, but renewals under it begin in April 2027 (agency Q&A: a January 2027 redetermination is evaluated under current rules). The provider page notes changes are 'not currently in effect'.Sources: 1 2 3 4 (Ohio's own pages, listed below) · checked 2026-10-20
Whether your own word counts in 2027
Limited. Medically frail self-attestation is accepted only at Presumptive Eligibility (Qualified Entities), and documentation is required at full application. Otherwise the agency lists documents for each exemption and activity (doctor's statement, letters from organizations, pay stubs). A pregnancy 'Verbal or Written Statement' is accepted. No 2027 self-attestation for volunteer/hours/student published.Sources: 1 2 3 4 (Ohio's own pages, listed below) · checked 2026-10-20
How many months it looks back
Application: 1 (month before application). Renewal: at least 1 month since last application/renewal (lookback = time since last renewal).Sources: 1 2 3 4 (Ohio's own pages, listed below) · checked 2026-10-20
How often you renew
6 months beginning 2027-01-01 per agency (WCER renewals start April 2027).Sources: 1 2 3 4 (Ohio's own pages, listed below) · checked 2026-10-20
How it checks medical frailty
Claims-based ex parte at renewal: one claim with a qualifying ICD-10 diagnosis (primary or secondary) is enough. A published Medically Frail ICD-10 code list (dated 2025-10-29). Provider documentation for new applicants or when claims lag. 3-tier model not mentioned.Sources: 1 2 3 4 (Ohio's own pages, listed below) · checked 2026-10-20

“If the individual has one claim with a diagnosis code indicating frailty, this will be sufficient to verify their exemption.”dam.assets.ohio.gov
