Ask for a fair hearing.
If your Medicaid was cut, stopped or denied, you can ask for a fair hearing. A hearing officer looks at your case again. The time to ask is short, so start today. Pick your state to see its deadline, how to keep your coverage while you wait, and where to send the request. Then fill in the letter and print it.
1. Your state
Pick your state and its deadline shows here.
The federal rule, in every state
- Each state must allow a reasonable time to ask for a hearing, not more than 90 days from the date the notice of action is mailed (42 CFR 431.221(d)). A state can allow less; the time on your notice is the one that counts.
- If the state sends the advance notice and you ask for a hearing before the date of action, the state may not end or reduce your coverage until the hearing decides, unless the only issue is federal or state law or policy (42 CFR 431.230).
- A state may reinstate coverage if you ask for a hearing not more than 10 days after the date of action (42 CFR 431.231).

2. Your request letter
Fill in what you know. Anything you leave empty prints as a blank line to write in by hand. What you type stays on this page: it is not saved, not sent, and gone when you leave or reload. Sign it, keep a copy, and write down the day you sent it.
Help with your hearing
A legal aid office can help you ask, get ready and speak at the hearing, usually at no cost if your income is low. Find legal aid near you (Legal Services Corporation). You can also call 211 and ask for legal aid.
You do not need a lawyer to ask. Someone you trust can help you. The state Medicaid agency and a hearing officer decide your case; this page only shows what the state has published.
Not legal advice. Each state line comes from that state's own Medicaid or hearing page, with the date it was read; the rules as data: appeals.json. If your notice gives a different date or address, follow your notice. Still on Medicaid and facing the work rule? Keep your Medicaid.
